Errors on Our Site
Prices and availability of products and services are subject to change without notice. Errors will be corrected where discovered, and Family Assurance Medical Plans reserves the right to revoke any stated offer and to correct any errors, inaccuracies or omissions including after an order has been submitted and whether or not the order has been confirmed and your credit card charged. If your credit card has already been charged for the purchase and your order is cancelled, Family Assurance Medical Plans will issue a credit to your credit card account in the amount of the charge. Individual bank policies will dictate when this amount is credited to your account. If you are not fully satisfied with your purchase, you may request a refund in accordance with Family Assurance Medical Plans’ Refund Policy.

Paying for Your Order
You may pay for your orders with major credit cards issue in the United States of America. Currently, we accept Visa®, MasterCard®, and Discover® Card. Credit and debit cards are charged prior to shipping any item(s) to you. By providing Payment information to Family Assurance Medical Plan by telephone or otherwise you have accepted recurring charges to the credit / debit card that you have provided. Such debit / credit card will be billed on a recurring basis as long as your Family Assurance Medical Plan remains active. If for any reason the debit / credit card used for payment of the family Assurance Medical Plan becomes inactive or has insufficient funds available when payment becomes due then you will provide to Family Assurance Medical plan another acceptable form of payment.
Validating Your Order
After you place an order using our shopping cart, we will check the information you give us for validity, by verifying your method of payment or shipping address. We reserve the right to reject any order you place with us, and/or to limit quantities on any order, without giving any reason. If we reject your order, we will attempt to notify you using the e-mail address you have given us with the order. Your credit or debit card will normally not be charged if we reject an order, but we will process a refund if the charge has been made against your card.
Your receipt of an electronic or other form of order confirmation does not signify our acceptance of your order, nor does it constitute confirmation of our offer to sell. The Family Assurance Medical Plans Web site reserves the right at any time after receipt of your order to accept or decline your order for any reason.
There are no Refunds for the Family Assurance Medical Plans after 30 days. During the 30 days refund period members will be refunded the difference after any disbursements such as commissions plus administrative fees, cost of marketing or other materials, shipping charges and membership fees for the first 30 days have been subtracted from the amount that was paid by the member.
We reserve the right to reject any order you place with us, and/or or to limit quantities on any order, without giving any reason. If we reject your order, we will generally attempt to notify you using the e-mail address you gave us when you placed the order. Family Assurance Medical Plans may, at its own discretion, limit or cancel quantities purchased per person, per household or per order. Family Assurance Medical Plans also reserves the right to reject any order you place with us. These restrictions may include orders placed by the same Family Assurance Medical Plans Web site account, the same credit card, and orders that use the same billing and/or shipping address. In the event we make a change to an order, we will attempt to notify you by contacting the e-mail and/or billing address provided at the time the order was made. Modern Medical Plans reserves the right to limit or prohibit sales to dealers.
Family Assurance Medical Plans provides service for up to 6 family members residing in the same household, per account. Failure to abide by this limitation is grounds for immediate account cancellation without refund. If you require coverage for additional family members, please purchase additional memberships via our Enrollment page.
TeleMedicine & Discounts Provided by Third Party
Family Assurance Medical Plans is only a marketing reseller of a TeleMedicine product provided by Teladoc / Healthiest You plans. Usage of said product is further governed by Teladoc / Healthiest You Plans and their respective service providers’ Terms & Conditions. You may contact Teladoc / Healthiest You Plans with any issues related to the TeleMedicine services.
Termination of Use
Family Assurance Medical Plans may, in its sole discretion, terminate your account or your use of the Family Assurance Medical Plans Web site at anytime. You are personally liable for any orders that you place or charges that you incur prior to termination. Family Assurance Medical Plans reserves the right to change, suspend or discontinue all or any aspects of the Family Assurance Medical Plans Web site at any time without prior notice.
Jurisdction and Venue
The use of the Family Assurance Medical Plans Website and the terms governing the purchase & use of Family Assurance Medical Plans’s products & services shall be construed and enforced in accordance with the laws of the State of Florida. The undersigned hereby consents to and submits to the jurisdiction of the appropriate Florida State Court, and the undersigned further agrees that any action or suit against Family Assurance Medical Plans or its parent company shall be brought in the appropriate court within Broward County, Florida. The undersigned shall not raise in connection therewith, and hereby waives, any defenses based upon the venue, inconvenience of the forum, lack of personal jurisdiction, sufficiency of service of process in any such action or suit brought in Broward County, Florida.
The Family Assurance Medical Plan is a private health plan and is NOT Medical Insurance neither is it a qualified health plan under the Affordable Care Act (ACA).
Discount Plans and Roadside Assistance are provided by third party providers which are governed by their own terms and conditions and may contain additional limitations and restrictions that is not mentioned in the terms and conditions on the Family Assurance Medical Plans website.
The Roadside Assistance Benefit is provided by Roadside Protect. Whenever you, your spouse or children under 24 living at home need roadside assistance for your vehicle, call our toll-free number at 877-680-1475 twenty-four (24) hours a day and request dispatch service. We will arrange to send help to your disabled vehicle from a participating facility.
Covered Expenses All expenses covered under the Roadside Protect Program are limited to One Hundred Dollars ($100.00) for any single claim and include the following:
(a) Towing – When your vehicle is disabled due to mechanical breakdown, the Roadside Assistance Administrator will arrange for a Roadside Contractor to tow it to the nearest service facility of your choice up to the service expense equivalent of $100. Additional expense will be your responsibility to pay to the towing Roadside Contractor. (b) Flat Tire Assistance – A flat tire will be changed with your spare tire. If, for any reason, the spare is not usable, the lug nuts cannot be removed or the vehicle has two flat tires with one usable spare, towing will be provided in accordance with the towing provisions. (c) Fuel Delivery Service – An emergency supply of fuel of up to three (3) gallons will be delivered if your covered vehicle runs out of fuel. You will be responsible for the cost of fuel. (d) Lock Out Service – If your keys are locked in the vehicle, assistance will be provided to gain entry into the vehicle up. In the event the keys are lost and a replacement key is required, you will be responsible for the total cost of a new key. (e) Lost/Stolen Key Reimbursement – Members will be r reimbursed up to $100 for replacement of their lost/stolen vehicle keys. To receive reimbursement, members must mail the following items along with their membership information to Roadside Protect’s offices following the instructions in the ROADSIDE ASSISTANCE REIMBURSEMENT section below: • Their original receipt for the payment of key replacement service. • A copy of the automobile registration (f) Jump Start – Jump start service will be provided to start your vehicle. (g) Winching/Extraction – If your vehicle is stuck in a ditch, mud or snow, but it is accessible from a normally traveled roadway, service will be given to either tow or winch the vehicle. Dispatch coverage for winching is limited to $100; any expense incurred beyond $100 will be your responsibility to pay to the Roadside Contractor.
Roadside Assistance Reimbursement. If for any reason we cannot provide the benefits listed in this Program, you must obtain an authorization number from our dispatch center to use at the service provider of your choice. We will reimburse you up to $100 or the specific amount listed above in the covered expenses (whichever is less) upon presentation of the original paid service provider receipt.
